Our commitment
- We are required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your PHI
- Treatment: to coordinate your care with clinicians, pharmacies, and laboratories.
- Payment: to bill and collect payment for services and products provided to you.
- Health care operations: for quality review, clinician training, compliance, and business management.
- As required by law: including public health reporting, health oversight activities, and lawful legal process.
Uses that require your authorization
- Most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of PHI require your written authorization.
- You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your rights
- Request access to and copies of your medical records.
- Request an amendment to information you believe is incorrect or incomplete.
- Request an accounting of certain disclosures we have made.
- Request restrictions on certain uses and disclosures, and request confidential communications.
- Receive notification if a breach of your unsecured PHI occurs.
Complaints
- You may file a complaint with us using the details on our Contact page, or with the US Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
